Who is eligible for the Exercise on Referral programme?
You must live in Southampton (or surrounding areas) or be registered at a Southampton GP surgery. If you aren’t eligible, we will direct you to other services.
How long is the programme?
The programme runs for 12 weeks, providing you with structured, ongoing support to build and maintain a healthier lifestyle.
How do I get referred?
You can be referred by your GP or another healthcare provider, or you can self-refer.
What activities are included?
The programme includes gym sessions and swimming.
How much does the programme cost?
The Exercise on Referral subscription is £50 for 12 weeks. You can also purchase memberships at individual Southampton venues, including The Quays Swimming and Diving Complex, Bitterne, and Chamberlayne Leisure Centres.
What support will I receive?
Alongside the tailored exercise plan, you’ll receive regular follow-ups at 6 and 12 weeks to check your progress and provide additional support.
Who is not suitable for the programme?
The programme is not appropriate for high-risk individuals. If you have any of the following conditions, you may not be eligible:
• Unstable hypertension
• Unstable angina
• Unstable diabetes
• Severe coronary heart disease
• Unstable and/or severe mental health problems
• Pregnancy (if previously sedentary)
What happens after I've been referred?
Once referred, a friendly member of our team will get in touch to arrange your first visit. They’ll review your notes, chat with you about your goals, and conduct a screening. From there, they’ll create a personalised exercise plan that’s tailored to your needs.
What happens at the end of the 12-week programme?
At the end of the programme, we’ll carry out a final evaluation to assess your progress and discuss next steps. If you’ve adhered to your exercise plan consistently, you should see positive results! We’ll work with you to set new goals for regular exercise, helping you maintain your healthy lifestyle.
